Intake Process
We worked together to identify things we would like to keep doing, to retire, to start. These discussions brought up other great topics such as, how our department could better communicate what kind of services our team can provide and facilitate.
The Result : 5 Questions
After sorting the feedback and content we were able to determine Form Requirements for our Intake Form.
Keeping the questions simple and to-the-point to reduce the possibility of team members abandoning the form.
